Deterministic AI and Agent Guardrails

Consequential Data Engineering

Agent development, process automation, and document intelligence for organizations that cannot ship an unauditable agent: every workflow definition versioned, 100% of agent actions logged to an append-only, tamper-evident audit trail a reviewer can replay, and HIPAA Security Rule, FDA, and FedRAMP requirements designed in rather than layered on after a proof of concept.

What do deterministic AI and agent guardrail services build?

They build agents, process automation, and document intelligence for organizations that cannot ship an unauditable agent. Every workflow definition is versioned, and each agent workflow is designed so its actions are logged to an append-only, tamper-evident record a reviewer can replay.

Agent development, process automation, and document intelligence built for organizations that cannot ship an agent whose actions aren't auditable โ€” every workflow definition versioned, every agent action logged, compliance requirements (HIPAA, FDA, FedRAMP) designed in rather than layered on after a proof of concept.

The Customer Problem: Four Agent Failure Patterns

A guardrail that runs after the action

The safety check filters the agent's reply, but the tool call has already written to the system of record. The bad write is there before anything stops it.

A run nobody can replay

The prompt version, model and retrieved passages were not kept. When a decision is questioned, the team can only re-run the agent and hope it behaves the same way.

Answers without a source

Retrieval returns fluent text with no link to the passage it came from, so a reviewer cannot tell a grounded answer from an invented one.

Behavior that drifts after an update

A model or prompt change alters outcomes on cases that used to pass. Nobody re-ran those cases, so the change surfaces as a complaint.

What distinguishes a compliance-aware agent from a demo

A working proof of concept and a production agent for a regulated workflow differ in exactly the places a demo skips: who is accountable when the agent is wrong, what evidence exists that it behaved correctly on a given run, and whether a human reviewer can reconstruct what happened without re-running the system. This service is built around those three questions from the first design session, not retrofitted after a pilot.

Deterministic Guardrails
Compliance-aware response boundaries enforced before an agent action executes, not filtered after.
Human-in-the-Loop
Review checkpoints on every consequential decision an agent is not authorized to make alone.
Audit Traceability
Every agent action logged and reconstructable without re-running the system.
Compliance by Design
HIPAA, FDA, and FedRAMP requirements designed in from the first session, not layered on after a pilot.

Mechanics and Guardrail Contracts

Each point where an agent touches a system, a source or a person has a contract and a check. The checks are the queries in the explorer below.

Each interface, its contract and the check that shows it holds
Interface Contract Checked by
Agent to toolEach call is checked against an allow-list and the tool's input schema before it runs; a refusal is logged with the rule that refused itThe refused-call query (Panel 1)
Run recordWorkflow version, model id, prompt hash, retrieved passage ids and every tool call are written to an append-only log per runThe replayability query (Panel 2)
Retrieval to answerEvery answer cites the passage ids it used from your own corpus; an answer with no citation is withheld or flaggedA citation check on sampled answers
Consequential action to a personActions on an agreed list wait for a named reviewer, whose decision is logged with who and whenThe held-for-review rows (Panel 1)
Model or prompt releaseA fixed case set is re-run on every change and compared with the baseline before releaseThe regression query (Panel 3)

Run logs are designed to support the HIPAA Security Rule's audit controls standard (45 CFR ยง164.312(b)) and, in a FedRAMP environment, the NIST SP 800-53 audit and accountability (AU) controls. Whether a deployment meets them is decided in your own assessment.

Guardrail and Replay Explorer

Three steps a reviewer can re-run. Each panel says whether it is a public command or a synthetic illustration, and every query is SQLite.

Panel 1: Tool calls refused or held for review

Every tool call the pre-action check stopped or sent to a person in the last seven days, by tool and rule.

-- Synthetic illustration: tool calls the pre-action check refused or held
-- for a person in the last seven days, by tool and rule.
CREATE TEMP TABLE IF NOT EXISTS tool_call (
  call_id TEXT PRIMARY KEY,
  run_id TEXT NOT NULL,
  tool TEXT NOT NULL,
  decision TEXT NOT NULL CHECK (decision IN ('allowed', 'refused', 'held_for_review')),
  rule_id TEXT NOT NULL,
  decided_utc TEXT NOT NULL
);

SELECT tool,
       rule_id,
       decision,
       COUNT(*) AS calls
FROM tool_call
WHERE decision <> 'allowed'
  AND decided_utc >= strftime('%Y-%m-%dT%H:%M:%SZ', 'now', '-7 days')
GROUP BY tool, rule_id, decision
ORDER BY calls DESC, tool;

Synthetic illustration A TEMP stand-in with an illustrative layout, not a product schema; nothing is written. Allowed calls are logged too; this query lists only the ones that were stopped. Runs as written in SQLite 3.

Panel 2: Runs that cannot be replayed

Runs whose record is missing something a replay needs: the workflow version, the model id or the prompt hash.

-- Synthetic illustration: agent runs that cannot be replayed, because the
-- record lacks the workflow version, the model id or the prompt hash.
CREATE TEMP TABLE IF NOT EXISTS agent_run (
  run_id TEXT PRIMARY KEY,
  workflow_version TEXT,
  model_id TEXT,
  prompt_sha256 TEXT,
  started_utc TEXT NOT NULL
);

SELECT run_id,
       started_utc,
       CASE
         WHEN workflow_version IS NULL THEN 'no workflow version'
         WHEN model_id IS NULL THEN 'no model id'
         ELSE 'no prompt hash'
       END AS missing
FROM agent_run
WHERE workflow_version IS NULL
   OR model_id IS NULL
   OR prompt_sha256 IS NULL
ORDER BY started_utc;

Synthetic illustration A TEMP stand-in; nothing is written. A complete record makes a replay possible; it does not make a probabilistic model return the same text twice. Runs as written in SQLite 3.

Panel 3: Cases that changed with a new model

Regression cases whose outcome changed, or that were not run, when a candidate model or prompt replaces the baseline.

-- Synthetic illustration: regression cases whose outcome changed, or that
-- were not run, when a candidate model replaces the baseline.
CREATE TEMP TABLE IF NOT EXISTS eval_result (
  case_id TEXT NOT NULL,
  model_id TEXT NOT NULL,
  outcome TEXT NOT NULL,
  PRIMARY KEY (case_id, model_id)
);

SELECT b.case_id,
       b.outcome AS baseline_outcome,
       COALESCE(c.outcome, 'not run') AS candidate_outcome
FROM eval_result b
LEFT JOIN eval_result c
  ON c.case_id = b.case_id
 AND c.model_id = 'candidate'
WHERE b.model_id = 'baseline'
  AND (c.outcome IS NULL OR c.outcome <> b.outcome)
ORDER BY b.case_id;

Synthetic illustration A TEMP stand-in; nothing is written. The model ids are placeholders. A changed outcome is a case to review, not automatically a failure. Runs as written in SQLite 3.

Typical Engagements

These describe the shape of an engagement in each setting. They are not named client results.

Prior-authorization document intake for a payer

Faxed and uploaded clinical documents classified and summarized, with every extracted value citing its page and every denial-relevant decision held for a nurse reviewer.

Clinical trial document review with human sign-off

An agent drafts findings against the protocol; nothing reaches the trial master file until a named reviewer approves it, and each run can be replayed from its record.

Benefits correspondence triage in a FedRAMP environment

Incoming letters routed by an agent whose tool calls are limited to read-only lookups and queue assignments, with run logs kept for the AU controls.

Scoped Pilot: The 2-week agent accountability review

What the two weeks deliver

Week 1

Pick one workflow. Map each decision point, the guardrail that governs it and where a person must approve.

Week 2

Prototype a replayable run record for that workflow, then deliver a gap list against your regulatory requirements.

Boundaries

  • No production agent is deployed in the pilot; the output is a reviewed design, a prototype run record and a gap list.
  • Your data and prompts stay in your environment unless you agree otherwise in writing.
  • No outcome is promised: regulators and your own reviewers decide whether a workflow is acceptable.
